Welcome to our Information and Referral system – your easy access point to health and personal support services in your community. This system is referred to as Community Care Resources (CCR) for Central and Toronto Central CCACs and as Community Care Information (CCI) for South East CCAC. This site was created by Community Care Access Centres and developed in partnership with our Local Health Integration Network, our service providers and community partners. CCR/CCI is an information and referral website by which Service Providers can register and update their own program information. Service Providers are organizations that provide community-based health services, and personal and social support services that may stem from any level of government, the non-profit sector, the private voluntary sector, individuals in the community, as well as from the business sector. CCACs are mandated by the Ontario Ministry of Health and Long-Term Care to provide information about and referral to services available in the community, by providing contact and program information about those services. |
