If as a Service Provider you choose to register with, submit or view materials in this system, please refer to the Terms and Conditions. Information that is confidential or personal will be subject to the Freedom of Information and Protection of Privacy Act (FOIPP), Personal Health Information Protection Act (PHIPA), and other relevant legislation and aligned with the Fair Information Practices defined in the Canadian Standards Association's (CSA) Privacy Code. This material is intended for general information purposes only. The information provided on the CCR/ CCI website is updated by the listed agencies. Any content concerns should be raised directly with the listed agency. If you subsequently access a website from any of the community care agencies linked to our site, you must refer specifically to their Privacy Policy and security measures with respect to your general information or personal health information. The participating CCACs are not responsible for information disclosed on any community agency website. Information Collection and UseThe system collects personally-identifiable information, such as your name, e-mail address, phone number, or postal code when you register with the participating CCACs. Personal information is used to correspond with that user about their registration information or user profile. We may also use the information to contact you about your opinion of satisfaction with our services or to inform you of updates to the system. Security - How do we protect your personal information?We have adopted processes and procedures to keep your personal information safe and secure. The personal information you provide is secured using Secure Socket Layers ("SSL") encryption technology. We use this SSL technology to prevent information from being intercepted as it is transmitted over the Internet. Only when it reaches the servers is your information decrypted and stored on restricted access computers. It is important for you to protect against unauthorized access to your password and your computer by not sharing your passwords or writing them down. Confidentiality WarningThe system strives to ensure confidentiality in e-mail communication concerning community services. Nevertheless, without the use of encryption, the Internet is not a secure medium and privacy cannot be ensured. Internet e-mail is vulnerable to interception and forging. The participating CCACs will not be responsible for any damages you or any third party may suffer as a result of the transmission of confidential information that you make to the participating CCACs, or that you expressly or implicitly authorize the participating CCACs to make, or for any errors or any changes made to any transmitted information. Disclosure of Information to Third PartiesWe will not sell or rent your personal information to third parties; nor will we share your personal information with third parties unless your consent is obtained at the point of collection, or otherwise. We will not disclose your personal information. However, when required or permitted by law, we must disclose personal information to law enforcement, regulatory or other government agencies, or when we find that your actions in our system violates the Terms and Conditions. Access to InformationService Providers may access their personal and organizational information to review, correct, update, or change, at any time, by fax, e-mail, online or phone. Information SecurityWe have in place reasonable standards of technology and operational security to protect all information provided by visitors from loss, misuse, alteration or destruction.
